- Application process for many health programs for adults and children.
- Avoid standing in long lines at the Department of Social Services Office.
- Bring the following documents on your appointment:
- Identification (CA ID, Consulate ID)
- Birth Certificate for person applying
- Social Security cards
- Proof of Income (check-stub, Tax Return, etc.)
- Proof pf Address (utility bill, phone bill, water and power bill, etc.)
