LSLC Instructional Materials Request Form
The Local School Leadership Council has a limited budget each year to purchase instructional materials.
Prior to submitting your materials request, please check with your Department Admin and/or your SLC Admin to determine if there are funds available for your request through other budget lines. If your AP advises that your request should go to LSLC, please complete this form. The LSLC meets once per month to review and vote on purchase requests.
